Envoyez le formulaire dûment rempli par courriel à info@cctx.ca, et notre équipe vous guidera au fil des étapes suivantes.

Les demandes sont approuvées par le comité d’adhésion du conseil d’administration. Tous les demandeurs signent notre entente de services qui décrit les conditions d’adhésion et de confidentialité conformément auxquelles les membres s’engagent à traiter toute information qu’ils reçoivent de l’ECMC.
